How to Create a List of Creditors

(also called the “Creditor Mailing Matrix”)

Easy:

Medium:

  • Email your List of Creditors to COB_Matrix@cob.uscourts.gov with your name, phone number, and case number, if known, in the subject line.
  • The list must be formatted as stated below.

Hard:

  • Mail a thumb drive or CD containing a saved copy of your List of Creditors (or a legible, printed copy) with your documents to U.S. Bankruptcy Court, 721 19th St., Denver, CO 80202.
  • The thumb drive or CD should be in a padded or protective envelope.
  • The list must be formatted as stated below.

Format

Prepare a Word document or Notepad text file (.doc, .docx, or .txt) listing your creditors and their mailing addresses in a single column, left justified, with two lines between each address. Each entry is limited to five lines. Do not include any symbols or special characters (|@#$%^&*()_+?). Do not include full account numbers or phone numbers. Do not list the amount owed.

How to Add a Creditor or Change a List of Creditors

Adding creditors or changing your list of creditors usually involves amending Schedules D and E/F too. Generally, you must file your new list of creditors, amended schedule(s), notice of amendment, and certificate of service, then mail copies of the amendments and the notice to your trustee and affected parties, and, finally, pay the fee. What to file when amending Schedules D and E/F to add creditors:

(1) the amended schedule;

(2) List of Creditors according to the above instructions (only the new creditors);

(3) the Notice of Amendment to Schedule, L.B.F. 1009-1.1;

(4) a Certificate of Service, L.B.F. 9013-1.2; and

(5) the filing fee, if any.
